For the fourth year in a row, we’re asking for your help in completing our congregational survey. This survey is exceptionally helpful to our staff and leadership teams. It is one way we gauge if our ministries fulfill our mission of being a loving community of disciple-makers serving and reaching generations with the gospel in our city and around the world. We care about your responses and how we can serve you better!
The survey will be live until March 11 and is available to anyone 16 years or older who considers FAC their home church. It should take under 10 minutes to complete!

This is an anonymous survey. Please answer as openly and honestly as possible.
Please take a clear position on each question. Even though there will be an option to answer “neutral” for each multiple choice question, please only choose this option if you are truly neutral and do not have any opinion on a specific question.
If you don’t believe you have enough information to answer a question please select “don’t know” or skip that question.
