This survey is exceptionally helpful to our staff and leadership teams. It is one way we gauge if our ministries fulfill our mission of being a loving community of disciple-makers, serving and reaching generations with the gospel in our city and around the world.

The survey will be live until Sunday, March 3 and is available for anyone 16 years or older who considers FAC their church. It should take about 8 minutes to complete! If you require a paper copy, please contact the church office.

• This is an anonymous survey. Please answer as openly and honestly as possible.
• Please take a clear position on each question. Even though there will be an option to answer “neutral” for each multiple-choice question, please only take this option if you are truly neutral and do not have any opinion on a specific question.
• If you don’t believe you have enough information to answer a question please select “Don’t Know” or skip that question.
